PLEASE READ THIS RELEASE FORM, SIGN AND FAX
ALONG WITH THE “REGISTRATION FORM” AND THE
"MEDICAL FORM"
BEFORE JUNE 1, 2010.
In consideration of the University’s acceptance
of my application to participate in the 2010
Guitar, Bass Guitar and Drumset Camp, I hereby
release the University of Miami, its officers,
and agents from any and all claims and causes
of action which might be brought by me or
my parents or dependents for loss of property,
personal injury or death sustained by me
arising out of any travel or activity conducted
by or under the control of the University
of Miami. I agree to abide by the camp regulations
and cooperate with camp authorities in furthering
the ideals upon which the camp was founded.
Because of the nature of the program, I agree
to remain for the entire week session unless
prevented by reasons beyond my control. I
understand that the University of Miami and
Guitar, Bass Guitar and Drumset Camp authorities
reserved the right to expel students/campers
for violation of University and Camp rules
and that under such circumstances no refunds
will be made. I understand that the University
of Miami and/or Guitar, Bass Guitar and Drumset
Camp will not be responsible for lost, stolen,
or damaged personal property of participants.
In addition, I understand that participants
are responsible for abiding by University
fire and safety regulations. I understand
that the $25 deposit is non-refundable and
the basic camp fee of $295 is non-refundable
after registration on June 14, 2010. No refunds
will be made on housing and/or meals after
registration on June 14, 2010. I understand
that all fees are to be paid in full by registration
on June 14, 2010 or a $20 late fee will be
assessed. I give Consent for the applicant
to participate in radio, television, audio
and video tape recordings and transcribed
programs without compensation and for photographs
taken at Camp to be used in Camp-approved
publications. I also give consent for the
applicant to participate in off-campus field trips.
